其正中畫有一壺，上書公廨夜祭四字，壺口有黃色圓形為襯底，旗上方寫加蚋埔，下括註泰山，壺下方為二支帶葉樹枝，四周帶框。

粉面金身木雕媽祖，座椅上，右手持鋁製芴板，底座前方漆寫：雲林東勢聖安宮三令聖母。

金身木雕觀音神像，胸前、額前、髮上各有一人工寶石，衣邊縐折及蓮花座勾勒紅漆，底座為綠漆邊。

皮製，袋呈黑色，二側縫以藤條，袋蓋呈半圓形，袋身有一安插袋帶的橫走皮條，該皮條二端縫紮在袋二側。

桌沿前方有雕飾，左右分別為八仙圖，中間為財子壽圖，木雕圖像上方鑲嵌玻璃保護，桌腳雕有獸面及花飾。
